2011-2012 OCSOA Injury Report

This form must be completed when a player is seriously injured. "Seriously injured" is up to your discretion as a referee, but basically a player who is hurt and does not return to the field, or one that will seek medical attention for their injury should always be reported. The results of the form will be submitted to OCSOA's designated CIF liaison for further action. This form can only be completed by registered users of the website.
